PCF has provided DSP’s a 1099 hotline number if they are in need of a duplicate form or have any questions/issues with their form.
The number is 410-821-4552.
You will need to clearly provide the following information:
Name
BP#
SSN
Current Address
Phone Number
Request needed (whether a duplicate form is needed or there is an issue needing some attention)
Additionally, a form is also attached so you can complete it at the depot. This form can be scanned, emailed (dsp.comp@pcfcorp.com) or faxed (410-583-1578) to us.
We will begin sending out duplicates sometime next week. When we received the returned form, we will forward that to DSP's as soon as possible.
